RESUMO

The purpose of the article: Generalized pustular psoriasis (GPP) is a rare auto-inflammatory disease. Patients with GPP may develop life-threatening complications, including sepsis, acute renal failure, neutrophilic cholangitis, high-output congestive heart failure, acute respiratory distress syndrome and death. The therapy of GPP is very limited and the course of the disease is unpredictable.Materials and methods: We report a 60-year-old woman presenting with widespread and confluent erythematous-desquamative plaques with numerous small pustules covering almost 70% of the body surface area. Over the past years patient had undergone different types of conservative treatment regimens including topical therapy, acitretin, cyclosporin, methotrexate and long-term treatment with systemic corticosteroids. Considering the patient's overall clinical condition, we proceed to initiate the biologic therapy with guselkumab.Results: Guselkumab (anti-IL-23) in the standard dose of 100 mg was administered subcutaneously at weeks 0, 4 and followed by a maintenance dose every 8 weeks. The remission of GPP was observed already after 12 weeks of treatment. The maintenance treatment in the period of 18 months shows stable clinical response.Conclusions: Our results support the evidence that guselkumab could provide an effective therapeutic approach in the treatment of GPP.

Seborrheic Dermatitis (SD) is a chronic and relapsing inflammatory condition accompanied by erythema, flaking and itching. Dandruff is a milder form of SD. Selenium disulfide (SeS2) is beneficial in both conditions. OBJECTIVES: to assess the efficacy of SeS2-based shampoo in dandruff/SD. MATERIALS & METHODS: an international, multicenter, observational study was conducted in 1407 adult subjects. SeS2- based shampoo was used 2 to 3 times per week for 4 weeks. Clinical efficacy criteria included improvement of hair, flaking, erythema and irritation on a scale from 0 to 5; investigator satisfaction was assessed at month 3 from 0 to 10. The subjects rated the impact of their hair problem (0 to 5), efficacy and satisfaction with SeS2-based shampoo at month 3. Tolerance was assessed throughout the study. RESULTS: At baseline, 68.2% had a flake severity score of 3-5, 49.9% an erythema severity score of 3-5, and 46.2% an irritation severity score of 3-5; 28.2% were bothered and 41.5% were very bothered. At study end, SD/D had at least clearly improved in 89.6% of subjects. All clinical signs had significantly (p<0.001) improved. 95.6% of subjects reported less pruritus. Significantly fewer (15.4%, p<0.0001) subjects were still bothered. The overall subject satisfaction score was 8.9±1.4; 97.6% of investigators were satisfied or very satisfied. Tolerance was good or very good in 98.8% of subjects. CONCLUSION: A SeS2-based shampoo applied 2 to 3 times per week is efficient and well tolerated in dandruff and SD.

INTRODUCTION: Seborrheic keratoses are very common benign epidermal tumors. Despite the high frequency, the pathogenesis is still unknown. They are extremely rare in the genital area. The participation of human papilloma viruses (HPVs) in pathogenesis of seborrheic keratoses is being discussed. AIMS: The aims of this case report are to inform about extremely rare lesion in genital area in a young man and evaluate the association of HPVs in the development of seborrheic keratoses. METHODS: We used histopathological examination to establish the correct diagnosis, which revealed signs of seborrheic keratosis. The real-time polymerase chain reaction method confirmed low-risk HPV 6 from the lesions. MAIN OUTCOME MEASURES: HPVs may play a role in pathogenesis of seborrheic keratoses. RESULTS: The patient was successfully treated with shave excision under spinal anesthesia. Six-month follow-up was without any recurrence. We suggest that HPVs can be considered as etiologic factor in creation of seborrheic keratosis. CONCLUSIONS: Seborrheic keratoses are very common on sun-exposed skin, but they are rare in the genital area, such as on the shaft of penis. This localization may lead to misdiagnosis. Seborrheic keratoses in genital area might negatively influence the sexual life of the patient. Containing HPV 6 low-risk virus, they never lead to malignant transformation.
